Appointments
We know you have many choices when choosing a dentist in Chicago, IL so we have made requesting an appointment a simple process via our website. Or, if you prefer, you may call our office and we will help you schedule an appointment that works best for you.  We value your time and you will see that we will generally see you right at the appointment time you are given.  In order for us to be efficient with the time that we have scheduled for you, we ask that you always give at least 24 hours notice if you are unabe to keep a scheduled appointment.  We also give you reminder phone calls 2 days before your appointment and ask that you kindly confirm your appointment with us.

Insurance and Billing
We accept most traditional insurance plans and PPO insurance.  We are providers for Delta Dental Premier and Cigna Radius.  Fell free to contact our office to verify acceptance of your plan. Lake View Dental Associates does not participate in Health Management Organizations (HMOs).  Our usual procedure is to ask our patiets to pay their dental deductible and any co-pay at the time services are rendered.  We will file your insurance claims for you, and direct the payment for your services to our office.  Utilizing this format, we minimize your out-of-pocket obligation, and make your financial life as easy as possible.  We accept checks, cash or credit cards. We also offer a flexible payment plan. If you have any further questions, our office manager, Juliette, or financial coordinator, Ambreen, are available to help you.
